Nurse Notes

A valid physical exam form is REQUIRED by NYS Mandate for the following students: all students in 9th grade and 11th grade, AND any students that are NEW or returning to the Nyack Public Schools District.   A valid SPORTS physical is also required for any student that wishes to participate on a Nyack High School Sports Team.

A valid SPORTS Physical is one that was performed 1 calendar year from the beginning of the month of the particular sports season. For instance, the Fall Sports Season officially begins on August 16 for football and August 24, 2026 for all other teams. Therefore, per NYS Athletic Regulations, any physical dated on or after August 1, 2025 is considered a valid physical for Fall.  ***Please note: a physical that was considered valid for Fall season MIGHT NOT necessarily be valid for the Winter or Spring seasons and MIGHT NOT be valid for the NYS Mandated requirement.***  

• The best thing to do is to keep your annual exam with your own pediatrician and to turn in a physical form of that visit to the School Nurse as soon as it's complete. Physicals by your personal physician will always be most thorough and are best for renewing prescriptions, annual Emergency Action Plans, and keeping up with immunizations. They are also covered by Health Insurance.
• Physical exams can also be obtained at most local Urgent Cares or Minute Clinics, and most accept insurance (always call ahead and ask if your insurance is accepted first).
• For those without health insurance, the District does arrange a series of free In-District Sports and NYS Mandated physicals throughout the school year with our District Doctor. Please inquire with the School Nurse regarding when the next District Physical day is to be put on the list.

• Additionally, if you would like your student to be able to receive medications in school, please fill out the top of these forms and have the doctor fill out (initial, sign, stamp) These forms should be renewed annually:
  • Standard Medications in school (tylenol, ibuprofen, benadryl, self-carry inhalers and epi-pens):  Medication and Intervention Consent Form 2026-2027.pdf
  • Other Medications not listed (i.e. allergy medication, ADHD meds, anti-seizure meds):  Medication Order Form - Non-formulary meds.pdf   *NOTE: these medications must be supplied to the Nurse in their original packaging; prescription medications should have the student and doctor's name on them.*
• Also, if your student has any medical issues such as asthma, food allergies that require an epi-pen, seizure disorder, or diabetes, they should have an Emergency Action Plan completed annually by the doctor. Your student should carry their emergency meds/testing equipment at all times and/or keep doses/equipment in the Nurse's Office:
